Does anybody have a document template for UPUs CN37 Delivery bill surface?

Hi all, for a customer of mine I need to create a UPU-document called "CN37 Delivery bill surface", as defined in this document on page 261. Before diving into the hard labour of creating such a document ourselves, I am hoping that any of my friendly forum colleagues can provide me with it. Would love to have a copy of the template. If you have one, please in Studio Pro, do an export so you get something like “CN37DeliveryBillSurface.mpk” and mail that to MendixAddons@tieka.nl. I’ll be forever in you debt.
