I am trying to connect to a SQL database using the above module from the App Store. When the database type is SQL, there is no option to enter a database name. How does the application know which database to connect to? A colleague has used a previous version of the module, which did allow a database name to be entered. Why has this been removed and how do I specify the database?

Check the form Database_New in the Database replication module.
There are some hide-when's specified based on database type. Perhaps something is wrong there?
You could consider updating to a newer version of Mendix so you can use the new module versions?
