I would like to add a Calendar (widget) to a form (appointments). Where do I find a list of optional widgets that I may use for design?

The Mendix app store is not available yet as well as the public 2.5 release. You should wait for the upcoming app store. The new 2.5 release will have an integrated app store in the modeler which enables you to search for widgets like the Calendar.

You can request widgets here. The Calendar widget is already requested by Johan den Haan.
