Checkbox / enumeration values word document.

I would like to be able to generate a word document with (in-word) editable checkboxes and enumerations. A default value should be given, coming from the value of the attribute of the object which is in the document template.

Here's an example of a word document which has this option. The list which is shown should be generated from an enumeration from Mendix.
